What is "case management"?
Case management is a highly structured process that will increase the recovery success rate of the individual and family members utilizing the service.
The purpose behind the development of “case management” comes from years of experience working with clients, families and treatment centers.
When you think about it, the twenty-eight days spent in a treatment center is just the beginning when you consider that many clients have been drinking or using for many years.
Even the best treatment centers cannot address all the problems or issues adequately in such a short period, and that is one of the disturbing reasons why many people relapse.
Traditional continuing care plans of the past have laid the groundwork for the now current case management services.
When clients or families contract for case management services, the possibility of long-term recovery is much greater than with these traditional continuing care plans.
Helping Hand Interventions and Consulting Services will continue to work with the client and their family far beyond the initial treatment experience.
When does case management begin & how long does it last?
Case management services can begin at various times during the treatment process, however, the ideal time to start is with the initial consultation to determine the level of treatment recommended.
Contract periods range from three months to two years. Once the client is engaged in a treatment setting, the case manager will work with the primary therapist and family members to structure the treatment goals and objectives for all involved.
A comprehensive case management plan is then developed for the client and involved family members.
The Case Manager will make revisions to the plan as needed with input from the participants.
Case management process
During the following months after the initial treatment experience, the case manager will work with the client and family to ensure the plan is followed.
In order for this to happen, the primary alcohol and drug counselor and family continuing care counselor will communicate with the case manager.
The case manager monitors everyone’s progress to make sure all participants are moving toward the treatment goals and objectives.
The case manager will monitor the attendance of all 12-step meetings, ensuring that a sponsor is obtained, a 12-step support group is established and that the 12 steps are a part of the recovery program.
Communications
During the process, case management communications between all the parties will primarily be by phone and e-mail on a weekly basis.
Every three months, the Case Manager will conduct a meeting with all participants.
The purpose of this meeting is to review the goals and objectives of the plan and address any issues that may have come up during the previous time period.
The longer a client and family are engaged in treatment, the higher the recovery success rate will climb.
